2013 Kindergarten Registration Day

A child will be admitted to kindergarten if he/she is five years of age by August 31, 2013.
In order to complete registration, please bring with you:
1. Immunization Records*
2. Birth Certificate (or equivalent)
*Washington State Law requires all children entering kindergarten to provide written evidence of immunity to diptheria, tetanus, pertussis, polio, measles, mumps, rubella, hepatitis B, and varicella according to the following minimum schedule:
Hepatitis B:
3 doses: 2nd dose must be given 1-3 months after the 1st dose.
3rd dose must be given at or after 24 weeks of age and at least 2 months after 2nd dose,
3rd dose must be given at least 4 months after the 1st dose
DTaP: 4 doses of DTaP. If the last dose is given on or after the 4th birthday
Polio: 3 doses. If the last dose is given on or after the 4th birthday
MMR: 2 doses MMR, both given on or after the 1st birthday and at least 28 days apart
Varicella: 2 doses. Must be given on or after the 1st birthday (4 day grace period applies) PARENT
REPORTED HISTORY OF DISEASE IS NOT ACCEPTABLE.
